Blank printable activity and exercise log with session entry blocks for activity type, duration, and energy level notes

Activity / Exercise Log

Blank daily movement record.

The Activity / Exercise Log is a blank daily movement record printable designed for students in grades 2–8 to track each individual exercise session — type of movement, start time, duration, distance or reps where relevant, and a short energy note. Unlike a weekly fitness tracker that gives one row per day, this log dedicates a full section to each session, making it ideal when students complete more than one workout in a day or want detailed notes on a single sport practice. PE teachers assign it alongside a unit on endurance, strength, or flexibility so students see their own progress session by session. SEL-focused educators pair it with a brief mood-before/mood-after check to illustrate the emotional benefits of movement. Athletes, dance students, and active kids fill it out independently as a personal training diary.

How to use this template

  1. Download and print the Activity / Exercise Log; students keep it in a PE folder or planner.
  2. After each session, fill in the activity name, start time, and total duration while it is fresh.
  3. Record any measurable detail — laps swum, sets completed, distance run — in the notes field.
  4. Rate energy level before and after using a simple 1–5 scale or emoji.
  5. Review entries weekly to spot trends and adjust goals for the following week.

Classroom & home ideas

  • Sport season log: athletes record every practice and game throughout a season to see volume load over time.
  • Fitness unit capstone: students compile a month of logs and write a short reflection on their most improved area.
  • Dance or martial arts classes: students log each studio session alongside school PE, giving a fuller weekly picture.
  • Step-challenge integration: add a steps column and cross-reference against a pedometer reading each day.
  • Parent conversation starter: students share the log at a family check-in to discuss after-school activity habits.

Frequently asked questions

How is this different from the Fitness Tracker Template?

The Fitness Tracker gives one row per day in a compact weekly view. The Activity / Exercise Log gives a dedicated entry block per session, so students who train multiple times a day or want detailed session notes have more space.

How many sessions fit on one printed page?

The default layout fits 5–6 session entries per page. Print multiple pages to cover a full week or training block.

Can a student use this log for non-sport activities like yoga or walking?

Yes — the blank activity-type field accepts any movement. Students write exactly what they did, whether that is yoga, a brisk walk, or a team sport.

Is there a way to set goals on the same sheet?

The top of the log has a blank header section where teachers or students can write a session goal or intention before starting, then compare it to the actual outcome at the bottom.
