SMART Goal-Setting Template
Goal plus steps frame, blank.
The SMART Goal-Setting Template is a blank printable frame that walks students through the five SMART criteria — Specific, Measurable, Achievable, Relevant, and Time-bound — so they can set a well-structured personal or academic goal from scratch. Aimed at Grades 3–8, it suits both classroom goal-setting sessions at the start of a term and individual check-ins mid-year when students need to reset their focus. Teachers use it during advisory periods, PBIS programs, and growth-mindset units. Students fill in each labelled section independently, then use the steps frame below the criteria to map out concrete actions. Older students in Grades 6–8 appreciate the scaffolded structure because it pushes them beyond vague intentions toward plans that are genuinely trackable. Counselors and parents also reach for this template when coaching a child through a new habit or project.

How to use this template
- Download and print the template — one per student or one per goal-setting session.
- Introduce or briefly review the SMART acronym so students know what each letter stands for before they begin.
- Students write their goal in the top section, then work through each SMART field in order, answering the guiding prompts in their own words.
- In the steps frame below, students list two to four concrete actions they will take and add a target completion date.
- Store completed sheets in student portfolios or post them where they are visible as daily reminders, and revisit them at the end of the set time period.
Classroom & home ideas
- Term-launch goal sprint: distribute the template on the first day of a new marking period and have students set one academic and one personal goal, then share with an accountability partner.
- Advisory or homeroom ritual: use the template at the start of each month; students compare their new goal to last month's and note what changed.
- Reading or writing workshop: let students set a specific word-count or book-count goal for the quarter using the SMART fields as a planning scaffold.
- School counselor session: counselors keep a stack on hand for one-on-one sessions when a student needs to reframe a challenge as an actionable goal.
- Parent-teacher conference prep: students complete the template before conferences so they arrive with a clear goal to discuss with their parents and teacher.

Frequently asked questions
What grade level is this SMART goal template designed for?
It is designed for Grades 3–8. The labelled SMART fields provide enough structure for Grade 3 students working with teacher support, while still being useful for independent middle-school learners.
Does the template explain what SMART stands for, or do I need to teach that first?
The template labels each section with the full word (Specific, Measurable, etc.) and includes a short guiding prompt inside each field, but a brief class introduction to the acronym before students begin will produce much stronger entries.
Can students use this template for goals outside of school, like a sports or hobby goal?
Absolutely. The blank format is subject-neutral, so students can apply it to any goal — reading a book series, improving at a sport, or building a new daily habit.
Is there a digital version, or is this print-only?
The template is provided as a printable PDF. For digital use, you can open the PDF in an app that supports annotation (such as Notability or GoodNotes) and type or write directly on the page.
